	Doctor’s Statement All children must have a doctors statement of health on record to attend.
	Medication Permission Form This form is required for administration of medication at Mini Texans. This form requires a doctors signature on prescription medications only.

Automated Tuition

	Bank Account Authorization Use this form to pay your tuition directly from your checking or savings account.
	Credit Card Authorization Use this form to pay your tuition through your credit card.
	Withdrawal Authorization Use this form to withdrawal from Tuition Express.
